With Miranda having been ruled out with a knee problem, Nerazzurri coach Roberto Mancini has elected to pair former Cardiff City midfielder Medel with Jeison Murillo at the heart of the defence.
In a boost for Inter, Mauro Icardi has been passed fit to start, after missing the past two games through injury, and will line up alongside new arrival Ivan Perisic and summer signing Stevan Jovetic.
As for Milan, Ricardo Montolivo returns in midfield in the absence of the injured Andrea Bertolacci, while Ignazio Abate slots in at right-back, with Mattia De Sciglio switching to the left-hand side due to the unavailability of Luca Antonelli.
